Of course, in the fight against cancer, one can never be completely sure of success. Sometimes, the removed tumor delays the disappointing outcome for several months, in other cases it gives the body several years, and sometimes even allows you to live until old age, not remembering that the person once had cancer. And there are more and more cases of complete victory over oncology! Doctors in this case use the cautious term "remission", allowing for the potential for the disease to return after the end of the favorable period. Moreover, in the medical environment it is generally accepted that a patient who has lived 5 years after the removal of a malignant neoplasm has a high chance of a future life. Of course, a 5-year remission period does not guarantee that the patient will not die of cancer in the coming years, but this good sign inspiring hope.

Currently, doctors distinguish several types of cancer, in the fight against which significant progress has been made, which means that they can actually defeat ailments. Let's get acquainted with them.

1. Prostate cancer

As a rule, prostate cancer develops very slowly, if not at all. This allows you to identify the disease at an early stage and make the right decision about how to deal with the tumor. Survival for 5 years after the discovery of oncology is almost 100%, and often the doctor chooses a waiting strategy, which means he decides not to touch the tumor, constantly monitoring it and operating on the prostate only if the disease progresses.

At the same time, it should be noted that in the case of metastases, prostate cancer is extremely difficult to treat. In such a situation, only 28% of cancer patients survive the 5-year milestone. Fortunately, metastases in this type of oncology are quite rare, and regular examination allows you to "catch" cancer at a time when it is still treatable.

Does testing help?
In order to timely determine the appearance of prostate cancer, medicine offers 2 ways to check - a rectal examination by a proctologist, in which a specialist examines prostate through the rectum, as well as a blood test for cancer cells (PSA test). By the way, the level of protein in a blood test, which shows the presence of oncology, can also increase with other diseases, which means that based on this examination alone, it is impossible to draw a conclusion about the presence of cancer.

2. Thyroid cancer

A very common type of cancer is thyroid cancer. There are several types of oncological lesions of this small organ. endocrine system, among which the papillary type is the most common. It develops very slowly and it is thanks to this that doctors are able to identify and remove the neoplasm in a timely manner. Statistics confirm that early detection in 98% of cases provides a 5-year survival rate for patients. True, after removing part of the "thyroid gland", the patient is prescribed hormonal preparations to be taken for the rest of your life. But is it really a reason to be upset when life is at stake?

It should be noted that not all types of thyroid cancer are so “compliant”. So, for example, anaplastic thyroid cancer develops rapidly and provokes the formation of metastases, which means it reduces the likelihood of survival within 5 years from the moment the disease is discovered to 7%.

Does testing help?
It is worth noting that screening tests to detect thyroid cancer in the early stages simply do not exist. This means that the only way to detect a tumor early is to regularly visit an endocrinologist, who can detect cancer by palpation or ultrasound. The person himself should be attentive to his body and consult a doctor, noticing a nodule in the neck or having a problem swallowing while eating.

3. Testicular cancer

This not the most common type of cancer is quite successfully treated by removing the testicle on which the tumor has formed. With such an operation, a man has one testicle, which means that his childbearing function is preserved and does not prevent him from having offspring. If testicular cancer is detected at later stages, only one surgical removal the problem cannot be solved. Chemotherapy or laser radiation will be required. In addition, 40 years ago, scientists developed the drug Cisplatin, which today helps to successfully fight even with running form this disease. It is the availability of methods for successfully combating an oncological tumor in the last stages that distinguishes testicular cancer from other types of cancer. Be that as it may, the 5-year survival rate in case of detection of such a tumor is at least 93%, and even the fight against a severe form of testicular cancer gives 73% of patients a chance to live 5 years or more.

Does testing help?
Screening methods for diagnosing testicular cancer also do not exist. In this case, every man should take care of his own health and consult a doctor for any tumor-like formations on the testicles, as well as for suspicions that one testicle has become larger than the other.

4. Breast cancer

One of the most common types of cancer is breast cancer. And it is doubly gratifying that it is in the fight against him that doctors have made the greatest progress. Scientists have made significant progress in studying the mechanisms of development of this disease and methods for diagnosing it. The numbers speak for themselves: early detection of cancer allows 89 out of 100 patients to live at least 5 years from the moment the disease is detected. True, it all depends on the degree of development of the tumor, which progresses quite quickly, as well as on the type of cancer, because some of them are more susceptible to drug treatment , than others. For example, tumors that are sensitive to estrogen levels are well suppressed by taking medications that reduce the level of this hormone, while “triple negative” cancer is considered an aggressive form of cancer that practically does not respond to targeted therapy.

Does testing help?
Of course, regular visits to a mammologist and the passage of mammography and breast ultrasound are the most effective ways early detection of the disease. As a rule, women are recommended to have an annual examination starting at the age of 40-45.

5. Melanoma

This type of cancer, which affects the skin and mucous membranes, is considered the most insidious tumor, which most often affects young people aged 15-25 years. Fortunately, modern medicine has achieved encouraging success in the fight against this type of oncology. Perhaps the whole point is the early detection of the disease, which is easy to see on the skin with the naked eye. Due to this feature, the detected melanoma gives a cancer patient a chance to live more than 5 years in 91% of cases. Moreover, for this, doctors resort to the usual removal of a part of the skin affected by malignant cells.

The insidiousness of melanoma is that it develops very rapidly. And if you do not notice the tumor that has appeared or delay in contacting a doctor, melanoma is more likely than other oncological tumors to be fatal. As practice shows, only in 15% of cases a patient with melanoma that has spread to other organs survives a 5-year milestone.

Does testing help?
As practice shows, melanoma is more often detected by patients themselves, turning to doctors with questions about tumors or dark-colored neoplasms that appear on the skin, which were not there before. Typically, melanoma appears on the scalp, back, scrotum, or between the fingers. With any neoplasm that makes you suspicious, you should consult a dermatologist. I am glad to welcome you, my name is Tatyana Panyushkina. No matter how you answer this question, I want to assure you, we will defeat cancer! Let me tell you my personal story.

This year, in 2018, I turned 64 years old. But today I am much happier and more energetic than 20 years ago.

At the age of 42, I had to endure serious stress, from which my path to happy life and health. The situation was connected with the financial sphere and my personal life. I have a strong predisposition to cancer in my family, both on my father's side and on my mother's side.

Two of my brothers, two aunts, three sisters died from oncology, and my mother also had to endure this disease. She went through treatment for six months, received a disability group. But she practiced herbs and healed herself, so she lived long life for over 30 more years.

Return to yourself. At 42, I began to feel that something was wrong in the urogenital area. I had to endure all this for two years, but the pain intensified, the discomfort did not go away. I drove away bad thoughts from myself and did not think that it could be oncology. Therefore, no action was taken by me.

During this period, my mother passed away, the disease began to develop very quickly. I had to go to the doctors, I was brought to the hospital in an ambulance already in a semi-conscious state. The doctors performed an examination, a biopsy and gave me a terrible diagnosis. I was asked to have an urgent operation.

I had to agree and stay in the hospital. Then I had chemotherapy and long treatment and then disability. The standard cable car that many people have passed on.

These thoughts did not leave me all the first night when I was in a medical facility. At this time, I remembered my mother, who, after doctors, switched to self-treatment with the help of prayers, various techniques, herbs, conspiracies. She knew this from her grandmother, who was once a healer.

My mother told me that without such treatment she would not have been able to live so long. If something like this happens to me, so that I do not agree to the operation and chemistry. She did not have an operation, she only had to undergo chemotherapy. Mom said that it was too much. I remembered that in the old box there were notes of my mother and grandmother.

What have I achieved? I have already said that cancer is curable. This was confirmed: any disease (oncology is no exception) originates from emotions.

And this is the area of ​​life that lends itself to change on our part. This means that we can change our own attitude towards the disease. Even in the East they said that if you call your enemy by his true name, then you can rule over him. We should not be afraid of oncology and call it to win. Also in the East they say that while a person moves, he lives. We are moving, so cancer will be cured. As long as we live, it is possible to find a way out of any situation.

Today I am healthy. I think it's because I left the hospital on time. This is my personal decision. At that moment, I decided: I will never go to a medical institution, I will not go to a hospital or a polyclinic.

20 years have already passed, and I don’t remember any hospitals. I feel that my health has become even stronger than it was at that moment. Previously, I had a cold and even such a disease as hemorrhoids, I faced problems with the liver, large intestine, kidneys. Diseases overtook me periodically. Today it is not even necessary to remember this.

And I advise you not to lose heart, to use every opportunity provided by fate. No need to be afraid of life situations. As long as we live and breathe, we can handle absolutely everything. — Yes, there are — in children, these are tumors of the hematopoietic immune system. These diseases are in the first place. In children, the formation of immunity occurs, those cells that, according to biological rules, should exercise control over the body, are capable of undergoing tumor growth. Therefore, leukemias and malignant lymphomas make up the bulk of tumors in children. In second place in children are tumors of the central nervous system. These two groups of diseases account for 60% or more of all neoplastic diseases in children.

In addition, in children, rare tumors occur in various organs and tissues, and they are usually associated with a disturbed embryonic anlage at the time of the child's development. But this happens much less often, so when they talk about childhood cancer, all over the world, first of all, they mean acute leukemia.

According to Rosstat, last year the death rate from cancer in Russia increased by 4%, that is, the total number of cancer patients in the country is increasing. Is there a similar situation in pediatric oncology?

— Statistics is a subjective thing, I do not fully trust these figures. This is a very delicate question. Due to the fact that 25-30 years ago a slow but bright revolution in the field of cancer treatment began, cancer is slowly becoming a fatal disease. chronic disease which is to be treated. There was an accumulation of cancer patients. If earlier it was like this: fell ill - died, fell ill - died, but now people live for years. And due to the accumulation of people who do not die from cancer, it seems that the number of people who have this disease is increasing. This is the first reason. And the second - the detection of tumors has become higher. I will give you an example.

Why do people over 60 die? If you look at the statistics, you will see that people are dying from cardiovascular disease. This may be the immediate cause of death, but if they were opened and looked to see if there really are tumors, or, perhaps, with the help of special methods, they made such an assessment, then we would see that the main part there is tumor diseases. They may not have clinical manifestations, but the beginnings of these oncological diseases are.

Well, the third reason is that life expectancy has increased. This is a serious question. We now in our country have about plus ten years in relation to our parents. I remember my dad, who passed away much younger than I am now. I can say that in terms of my physical abilities, condition, and so on, this is a completely different life. Today, 70-year-old people are in working condition. The Russian government is now even considering giving these people the opportunity to work without keeping a pension for those who work, which is a sign that people are living. The increase in life expectancy also coincides with the increase relative to cancer.

But I want to focus on something else. The main thing is that today cancer is a curable disease. This is a very important issue, and the goal of the medical community is to meet the highest level, the highest standard that exists today.

In this case, how to achieve and maintain a high level of medical care? What, for example, is being done in pediatric oncology for this?

We have been doing this work in children's practice for many years. We have a range of tools for this. The first such tool is very important - it is the unification of doctors into a professional community that could use uniform standardized treatment protocols on the territory of the Russian Federation, which is very important for a child in our large country from Vladivostok to Kaliningrad to receive the same treatment methods, moreover, that they were controlled.

To do this, over the course of 25 years, slowly, step by step, such cooperative groups of multicenter studies have been created that are doing just that. And I must say that in this situation we have achieved amazing results: living in a country that belongs to the group of developing countries, we have achieved the results of developed countries in the field of pediatric oncology. This was recorded by the World Health Organization at a special meeting of the WHO, where Russia presented its results so that we could broadcast them to those countries where the results are even lower, for example, to Asian countries.

In India, for example, there are 280 million schoolchildren (this is more than the population in our country and, perhaps, in the surrounding countries), and the survival rate for children from cancer is approximately 10%. And here in the Russian Federation, depending on the various forms of cancer, it is almost 80% - from 70 to 80%.

But there are tumors in which there is a higher survival rate with the correct formulation of the issue of diagnosis, with interdisciplinary interactions, the participation of doctors of different specialties, the management of these patients, access to drug therapy is optimal, and so on. There are so many state conditions that we, Russian Federation I think we have achieved here.

Every month, a team of specialists travels to various capitals of the constituent entities of the Federation to carry out relevant work there, namely, to consult patients, conduct training lectures for doctors, including doctors of various specialties, meet, if necessary, with undergraduates and university professors, if there are universities there, hold meetings in oncology dispensaries for adults.

In addition, we have organized in Moscow at the Dima Rogachev Center the so-called school of postgraduate education in our region, and therefore we are preparing personnel for our purposes. We are with great hope we look to the future, because our goal is to use all the innovations that appear in the world to treat our Russian children and not only Russian ones, because Belarus, Kazakhstan, Uzbekistan, Kyrgyzstan and Armenia have joined our community in the field of treatment.

And do you send those young specialists that you train to the regions? Why I ask: I often hear that there are not enough qualified specialists in the regions. All the same, everyone from the regions wants to go to Moscow for treatment.

- I'll answer you like this. How much do you think the healthcare budget in the United States is compared to Russia? So, the national budget spent on health care in America is larger than the national gross budget of Russia. Do you understand everything? No questions? No. We will not discuss this issue, because medicine in America is very expensive, many orders of magnitude higher than ours, and health care costs are directly reflected in the salaries of employees who work. The work there is very hard, but the salaries of the entire medical class, the entire medical class are also very high - salaries are higher than the average wage in America. You probably have a good idea. We do not have this, so there was a certain outflow of people who were able to work from this industry. This is the first.

There is such an internal opinion that Israel is better. Israel uses American technology, but Israel is our own people. Our. With all the ensuing consequences.

Well, for example, we had such a case: a teenage girl developed a tumor of the calcaneus, and this is a very complex structure. We turned to our experts. Our specialists could not perform an organ-preserving operation, they suggested amputation to remove this tumor. We searched for and found such a specialist in Germany, who specializes in this. He came to us here and successfully operated on this teenage girl with our doctors. But this is a piece of work. That is, I want to say that one should not think that the average doctor in Russia, in the average version, is worse than the doctor abroad.

That is, each person and his cancer is individual, and the key to this cancer can be found, molecular genetic defects can be found, and there are drugs that need to be treated for them. Therefore, constant training is required, therefore people who work in our specialty are knowledge hunters. They have to do it in order to achieve results.

More and more new medicines, more and more new technology, doctors seem to be getting better, they have more and more knowledge. What is your forecast as an experienced person: what will happen to cancer in the next 10-20 years? Will it be possible to defeat this disease?

February 4 has been celebrated as World Cancer Day every year since 2005. It was founded by the International Union Against Cancer (UICC) in order to draw the attention of the world community to this disease.

— I think that 100% will succeed. First, after 2000, the last 15-16 years have been very fruitful in the field of science. Managed to install genetic code person. Today, a complete assessment of a patient's genetic status can be made. Each person has his own cancer, even with the same name - stomach cancer, leukemia - it is a large palette different types, with different genetic disorders, for which it is impossible to find a uniform treatment.

I'll give you an example. We have such a disease indicative - chronic myeloid leukemia, in which patients live from three to five years. This tumor at the initial stage is supposedly benign in nature and amenable to some kind of treatment, but after some time the patient experiences a so-called blast crisis - a transition from a relatively benign process to a malignant one. All patients died until we found the molecular defect in this disease. Drugs have been picked up for this molecular defect, this medicine is now taken in tablet form by patients. The problem has completely disappeared. There is no need to treat the chronic phase of diseases, no need to use drugs, no need to use chemotherapy drugs, drugs, radiation, operations, hospitals. On an outpatient basis, a person receives treatment and achieves what is called molecular remission, that is, a complete recovery, and can live for a very long time. All forms of cancer are now on this path.

That was a long time ago. When not only magnetic resonance therapy was not in sight, there were no computers, mobile phones. And let's not talk about the Internet at all.

But cancerous tumors, like 290 million years ago, like today, affected many people. And although in terms of the number of victims, tumors were inferior to diseases of the heart and blood vessels, the fear of cancer is incomparable. And it just so happened - in all countries, including Russia - about a heart attack or stroke, the patient, his relatives were certainly informed, but the cancer diagnosis was accompanied by an unwritten taboo. Why? Is a cancer diagnosis the same as a sentence? Therefore, doctors often do not report the diagnosis to the patient himself, and the decision to speak or not to speak about the diagnosis is left at the mercy of relatives.

I will return, however, to the times when there was no Internet and mobile phones. My father, on the eve of his 69th birthday, this is 1972, began to suffer from pain in his stomach. We went around specialists, clinics, they conducted research on him, took endless x-rays. In vain. But another x-ray in the Moscow First City Hospital showed: extensive cancer of the stomach and part of the esophagus. Operation. They removed the entire stomach, part of the esophagus. And then an amazing phenomenon manifested itself, which is present, perhaps, only among oncological patients. So he knows that he is in the oncology clinic, he knows that he had an operation for cancer. Often also chemotherapy and radiation treatment. But in spite of everything, the patient somewhere in the subconscious does not believe in the diagnosis. My father, who managed to simply escape from the intensive care unit in which he was placed after the operation, justified his act: "Everyone there has cancer. But I have polyps. Why should I lie with them?"

I am not my father! - handed an extract from the medical history, which indicated what kind of cancer, and what kind of operation was performed, and recommendations. And through the entire certificate with a bold red felt-tip pen, the resolution: "Do not give it to your hands!" I hid this verdict in my closet so that my father would not see, so that he would not know. And here is another paradox. In our family, it is not customary to look into other people's closets. But the father not only looked in, but also found this very certificate among the linen. It would seem that everything became clear. He shouted, "I have cancer. I know." But somewhere, again, in the subconscious did not believe in it. Went to work. Stubbornly ignored the elevator, and climbed to the 6th floor on foot. He fried his own potatoes in lard. After such a meal, pain began to torment. Then he sat down on oatmeal and pureed soups. When once again they made a remark to him, they say, it is impossible to eat fried food after such an operation, he was indignant: "I love it." Father lived 83 years. He stopped working at 80. He did not forget to visit a hairdressing salon, where he cut his hair and did a pedicure with "his masters." He did not deny himself a glass of vodka or cognac, continued to smoke. Sometimes he could confidentially inform the interlocutor: "You know, I have cancer. You see how I lost weight, all the costumes are great." He stubbornly did not want to put on a new suit - there was a certain coquetry: to demonstrate that he "has lost so much weight because of cancer."

Time has changed. The indicators and the number of cancer patients are different now, and the possibilities of diagnosis, other results of treatment. But, as before, there is no recipe anywhere in the world for how to act when a tumor is detected. Yes, there is a tendency to inform the patient about the diagnosis. Moscow's chief oncologist Anatoly Makhson believes that a cancer diagnosis should not cause fear, that it should be perceived like any other. And in Russia it is even more acute because the amazing Russian mentality interferes with the situation with its age-old "maybe it will blow over." I can’t remember how many times it has been said, written that after 40 years, an annual visit to a urologist is mandatory. This is constantly repeated by the chief urologist of the Russian Federation Dmitry Pushkar. I am convinced that most of those over 40 will not remember when they were at the urologist. Especially men.

But to be completely honest, part of the blame is still for the health service. From a visit to the doctor, people are stopped by the impossibility to get qualified advice, effective help without any problems. And the farther from Moscow, the more such problems.

Each patient has his own situation, which cannot be ignored. And another problem: there is no trust in the doctor. They turn to him when they really feel like it. Yes, and the qualifications of specialists are sometimes such that cancer is missed. Because there are so many advanced stages of the disease. How many bitter confessions of cancer patients that they regularly visited doctors, and the tumor was found only at the 4th stage. How to explain this? However, it is not necessary to explain - measures must be taken.

Why did you decide to start a conversation about whether or not to inform the patient, his relatives about an oncological diagnosis? Why is it so important that public people began to talk about him openly more often? Yes, all for one reason: please take care of yourself! Of course, health is a special, very intimate sphere of life. Not everyone is able to "make public" failures in it. And if it concerns diseases of the prostate gland in men or cancer of the uterus or ovaries in women, even more so. Urologists, gynecologists are constantly faced with the fact that even the people closest to their patients do not know about the suffering of a husband, wife, mother, father. It is not uncommon to ask the physician not to disclose the true diagnosis to family members. How to act as a doctor? Tricky question? Does a doctor have to be a psychologist? Necessarily. But it is even more important that the entire system of medical care work for the patient, for the protection of his health. Unfortunately, we cannot boast of this.

An illustrative example. At a friend's double citizenship- Russian and Canadian. In Canada, she was diagnosed with breast cancer. Quickly, in an outpatient clinic, they examined and appointed the day and hour of the operation. The patient arrived early that morning at the clinic. And at one o'clock she was operated on. She still doesn't know who it is. Removed the breast. Discharged the next day. For some time we watched at home: a nurse came, the attending physician called. No complications. It was 8 years ago. She buried her husband 3 years before the operation. She got married shortly after the operation. Happy in a new marriage. Goes to the pool, travels. But at a certain time, she must be checked at the clinic where she was operated on. You can't miss. The help system does not tolerate violations.

As a medical observer, I am often asked: which clinic should I go to, which doctor should I contact? All for the same reason: there is no trust in the health service. The situation when the patient does not even know who performed the operation is simply nonsense for us. And if we are talking about oncology - even more so.

And one more thing, which is also not customary to speak out loud. Sometimes the treatment of a cancer patient ruins both him and his loved ones. After all, it costs from 30 thousand to tens of millions of rubles. Depends on the stage of the disease. The patient who has early stage tumors, as a rule, it is enough to operate, and he recovered. In such situations, 40-50, well, 70 thousand rubles is enough. Another thing is when advanced stage. When, in addition to surgery, it is necessary to apply radiation exposure, chemotherapy. The spending here is endless.

Most patients can get qualified help. But then it starts: basically, expensive drugs do not cure, but prolong life. It cannot be said that if the patient does not receive this drug, he will die. And if he gets it, he'll get better. There are pathologies that can be cured. For example, chorionepithelioma of the uterus. From this malignant tumor before 95% of women died. Now drugs cure 98%. Moreover, after such treatment, they can give birth. But this is a very rare disease. And if we take mass diseases, then basically everything depends on the stage, here we are talking about prolonging life. And this extension, especially when it concerns children, is very expensive.

Our health care, and not only ours, is not able to bear such a burden of expenses. That is why it is so important that public people not only speak out loud about their oncological diseases, but become actors, creators of various charitable foundations that help oncological institutions, specific patients. Cancer service cannot do without charity. Unfortunately, it is impossible to treat at the modern level without the help of philanthropists, only with state money.

Someday you don't have to be afraid to die. It's scary to die right now. Previously, it was believed that cancer was incurable, that it did not need to be treated at all. Today, says the director of the Cancer Center. Blokhin Mikhail Davydov, 60% are cured. And 40%?
